❄️ Inclement Weather Policy
Safety First: The safety of the Client and At Home Concierge Nurse, PLLC is the top priority. In the event of inclement weather (including but not limited to heavy snow, ice storms, flooding, or severe storms) that makes travel unsafe or significantly impedes the Concierge's ability to provide services, the following provisions apply:
Postponement or Cancellation: The Concierge may, at their discretion and in consultation with the Client, postpone scheduled services or cancel services altogether. Every effort will be made to provide reasonable notice of any changes.
Rescheduling: If services are postponed due to inclement weather, the Concierge will work with the Client to reschedule services as soon as reasonably possible.
Refunds:
If services are canceled due to inclement weather before any services have been rendered, the Client will receive a full refund of any monies paid.
Please refer to the Refund Policy section for further refund conditions.
Travel Delays: If inclement weather causes significant travel delays that impact the Concierge's ability to provide services on time, the Concierge will notify the Client as soon as possible.
💳 Billing and Financial Policy
At Home Concierge Nurse, PLLC is a PRIVATE PAY practice. We will not submit claims on your behalf to your health insurance provider. Payment is due a minimum of forty eight (48) hours before the date of service. If payment is not received prior to forty-eight (48) hours of service, your appointment will be canceled.
We will provide you with a receipt for services received from At Home Concierge Nurse, PLLC.
Acceptable Forms of payment: All major debit /credit cards.
AGREEMENT: By signing this agreement, I indicate that I have read and understand this Billing and Financial Policy. My signature below indicates that I accept this policy and agree to abide by the terms and conditions set forth herein.
🔀 Refund Policy
Cancellation by Client:
If the Client cancels more than seven (7) days before the scheduled surgery date, the Client will receive a refund of any monies paid, minus any applicable credit card processing fees.
If the Client cancels within three (3) days of the scheduled surgery date, the Client is responsible for fifty percent (50 %) of the total fee.
Extenuating Circumstances: At Home Concierge Nurse, PLLC may, at their sole discretion, offer a partial or full refund, minus applicable credit card processing fees, in the event of extenuating circumstances such as a medical emergency, natural disaster, or other unforeseen events that prevent the Client from proceeding with the surgery and utilizing the Concierge services.
Cancellation by At Home Concierge Nurse, PLLC: If At Home Concierge Nurse, PLLC cancels services due to unforeseen circumstances that prevent the Concierge from fulfilling the agreement, the Concierge will refund all monies paid by the Client, including any credit card processing fees paid by the Client, except in cases of Force Majeure (as defined below).
No Refunds for Services Rendered: No refunds will be issued for services already rendered, including but not limited to transportation, home visits, and any care provided.
Force Majeure: Neither party shall be liable for any delay or failure to perform its obligations under this Agreement due to any cause beyond its reasonable control, including but not limited to acts of God, war, terrorism, epidemics, pandemics, civil unrest, and governmental regulations.
